Independent Legal Entity Inspection and Testing Institutions

(Foreign institutions refer to this item for implementation)

"Application Form for Qualification Certification of Inspection and Testing Institutions" (1 copy, with electronic text in the format published on the Provincial Quality Supervision Bureau website);

Business license of legal entity, institutional legal person certificate, or other legal entity proof documents (business scope should include inspection and testing services);

In regions implementing commercial registration system reforms, business scope information must also be submitted (1 copy, original required for verification);

Organization code certificate (1 copy, original required for verification; in regions where the business license and organization code certificate are combined, submit the business license only);

Proof of fixed premises ownership/usage rights;

Proof of labor relations for professional technicians and management personnel, i.e., social security proof stamped by the social security department (1 copy, original required for verification, with three months or more of social security records);

Proof of independent allocation of inspection equipment (1 copy, original required for verification);

Typical inspection reports or certificates (one each; applicable for initial and expanded scope applications, additional copies may be required as per the review team leader's request);

Records of internal audits and management reviews of the management system (applicable for initial and reassessment reviews);

Qualification certificates for personnel engaged in special field inspection and testing.

All the above materials must bear the official seal of the inspection and testing institution (legal entity).

Non-Independent Legal Entity Inspection and Testing Institutions

In addition to the materials required for independent legal entities, the following additional materials must be prepared:

Legal entity authorization document (1 copy)

Approval document for the establishment of the inspection and testing institution (1 copy)

Appointment document of the highest management official (1 copy)

All the above materials must bear the official seal of the inspection and testing institution (legal entity).
