Mechanism of Action: Paliperidone is the major active metabolite of risperidone. The mechanism of action of paliperidone in the treatment of schizophrenia is unknown but is thought to result from a combination of antagonism at central dopamine D2 and serotonin 5HT2A receptors, as well as H1 histamine receptors. Paliperidone has no affinity for cholinergic or α-/β-adrenergic receptors. The pharmacological effects of the enantiomers (+)- and (-)-paliperidone are similar in vitro.
