When preparing laboratory CMA certification materials, non-independent legal entities must additionally prepare the following documents besides those required for independent legal entities:

Legal Person Authorization Document (1 copy)

Inspection and Testing Institution Establishment Approval Document (1 copy)

Appointment Document of the Top Manager (1 copy)

All the above materials required for laboratory CMA certification must bear the official seal. The materials needed for modification applications (such as address changes, name changes, legal entity nature changes, or cancellation of testing capabilities) vary case by case and are not listed here individually.
